A. Patients First Visit
When you arrive for your initial visit at COSPT you will be asked to provide the following information:
- Written physician referral form
- Insurance cards with ID and group number
- Patient social security number
- Patient Information Paperwork
- Co-payment if applicable
- List of medications currently taking
B. What Should You Wear?
- Please wear loose-fitting, comfortable clothing.
- You may be asked to change into a patient gown or pants for your evaluation and treatment, however if you are more comfortable in your own attire please bring your own shorts and t-shirt.
C. How Long Is Each Visit?
- Each visit will take approximately one to one and a half hours depending on your treatment program. The frequency and duration of your treatment program will be determined by your physician and physical therapist's recommendations and also based on your individual goals and availability.
D. What Should You Expect During Your Initial Visit?
- During your initial evaluation your physical therapist will review your medical history with you and will complete a thorough therapy evaluation. Together you will determine a treatment plan that meets your needs.
